Community Assessment & Planning

A community health assessment identifies and describe factors that affect the health of a community, and the factors that determine available resources to address those factors. A community health board, or another lead organization, collects, analyzes, and begins to use data to prioritize issues and make decisions.

Otter Tail County Public Health is part of the Partnership for Health Community Health Board which includes the counties of Becker, Clay, Otter Tail, and Wilkin.

Minnesota Department of Health has created the latest Statewide Health Assessment in 2017 and Mahube Otwa has also created a 2017 Community Needs Assessment for the counties of Becker, Hubbard, Mahnomen, Otter Tail and Wadena.
